AOA Scholarship of Excellence Nomination Form (2024)

Use this form to submit an application for the AOA Scholarship of Excellence Award. Provide all required information and upload all required documents in PDF format. All applications must be received by October 15, 2023. Complete all required data cells as required. You can save partially completed nomination forms. Just click on "Save and Continue Later" link at the bottom of the form, then follow the instructions, and enter your email to receive the continue later link. When all data has been entered and all documents have been uploaded, Submit by clicking on the Blue "SUBMIT" button at the bottom of the form. You will receive an electronic alert that your form has been successfully submitted. If you have not completed or uploaded all required data, you will receive an alert at the top of the form, alerts you that there is a problem with your submission and the alert will instruct you to "Review the fields below." The written essay, of no more than 500 words, should describe how the student applicant has contributed to, been involved with, or served the mission of an auxiliary organization(s) at their campus of origin or within the CSU system. This essay should articulate the student applicant's qualifications for receiving the award.
